Patient Information

Our office hours are 8:00 a.m. – 5:00 p.m. Monday through Thursday and 8:00 a.m- 4:00 pm on Friday.

**We are now offering telehealth visits for New and Established patients**

If you are a prospective New Patient and would like to make an appointment or inquire about the practice, please either call us at 919-240-7269 or send us an email at info@mosaiccarenc.com. Please provide us with your contact information, the best time to reach you for a call back, your insurance plan, and what type of care you are seeking.

If you are an established patient, please call us 919-240-7269 to make an appointment or request an appointment on the patient portal. Please let us know the reason for your visit so that we can schedule an appropriate time slot for your appointment.

We reserve acute appointment slots daily for urgent problems that require same-day evaluation. Please call the office at 8:00 a.m. to schedule a same-day appointment.

Please bring your insurance card and ID card to all appointments. Please download and complete new patient forms and annual visit forms prior to your appointment, or arrive at least 15 minutes early in order to complete these forms.

We believe that your time is valuable. We strive to stay on schedule and see each patient at the time of their appointment. Please arrive early for your appointment so that you can help us remain on schedule and keep wait-times for all of our patients to a minimum.

If you are unable to come to your appointment, please notify us at least 24 hours in advance so that another patient can make use of that appointment time.

Please bring your insurance card to all appointments, and contact your insurance company prior to your visit to determine required deductibles and copayments.

Our practice is in network with the following insurance companies:

• Blue Cross Blue Shield
• Aetna
• Coventry
• Cigna
• Duke Select and Duke Basic

* For 2020 Affordable Care Act Marketplace (healthcare.gov) participants: We are NOT in network with the new BCBS Blue Home plan, a UNC Health Alliance plan with a limited network. If you have a Blue Home plan and choose to seek care with us out of network, you will be charged our new reduced self-pay fees for office visits and we can assist in filing to your insurance for reimbursement.

We are NOT in network with United Healthcare, Medicare, Medicaid, Cigna Connect, or Ambetter for office visits. 

If you have an insurance plan that is not accepted at our practice, you will be charged our self-pay rates for services and will be required to pay in full at the time of the visit. For all out-of-network plans except Medicare, you may file for reimbursement from your insurance plan for out-of-network payments. We are non-participating providers with Medicare, which means that Medicare patients may not file for reimbursement for office visits. However, Medicare, United Healthcare, Blue Home and other out of network insurance plans can be billed by our in-house Quest Lab for lab testing.

Our office hours are 8:00 AM to 5:00 PM Monday-Thursday and 8:00 AM to 3:00 PM on Friday. When you have urgent health care needs that arise outside of these hours, please follow these recommendations:

• Life-threatening emergency situation: Call 911 or visit your closest hospital emergency room
• Acute medical condition or Injury: We have a co-management agreement in place with a nearby Urgent Care Center. Please be sure to indicate our practice as your primary care provider. This center has agreed to share the outcome of your visit with us so that we are aware of the treatment you have received and can provide follow-up services as needed. Our providers are available to these centers via phone in the event that medical record information is needed to inform their treatment decisions.

• Rapid development or worsening of symptoms that cause you significant discomfort or concern: Please call our office at 919-240-7269. You will be asked to leave a message for the doctor on call and one of our providers will return your call within one hour.
• We maintain a number of same-day appointment slots and encourage you to contact the office at 8:00 AM on our next business day for complete evaluation.

Behavioral Health Collaborators:

We partner with many local behavioral health providers, and we have co-management agreements in place with several of these providers, who are listed below.  We consistently communicate with our behavioral health partners about our patients’ health, and collaborate closely in creating their treatment plans. We create shared goals in the care of our patients, including addressing the connections between the body and mind.
